Reassessment Request for My Aged Care
A support plan review adjusts your existing support plan. A reassessment can change your classification. Start by talking to your provider; they can review your care plan and contact your assessor. You can also call My Aged Care yourself on 1800 200 422, or speak to an Aged Care Specialist Officer at selected Services Australia centres. My Aged Care takes these requests by phone, so use this letter as your script and keep it as a written record, and give your provider a copy.
General information, not financial, legal or clinical advice.
Before You Start
- Dates of falls, hospital stays or diagnoses.
- What daily tasks have become harder.
- What care is not being met now.
- A GP letter, if you have one.
The Letter
Anything in [square brackets] is a prompt to fill in. The copy, download and print buttons keep the brackets so you can find them.
What to Expect Back
The assessor may change the support plan or decide a reassessment is needed. If you had a Home Care Package, you keep your Transitioned HCP Level. You are approved for a Support at Home classification only if the reassessment gives a higher classification.
