Switching Providers: The Practical Playbook
It is allowed, it is free, and most providers do not put up a fight. Here is the order of operations that avoids gaps in care.
Under Support at Home, the participant chooses the provider. There is no exit fee, no claw back and no penalty. The participant's funding follows them.
Step 1. Pick the new provider. Use the My Aged Care directory and look for providers in your region that offer the services already in the care plan.
Step 2. Ask the new provider for a draft care plan and pricing. They will usually do this for free because it is a sales conversation.
Step 3. Notify the outgoing provider in writing. Email is fine. Ask for a final statement, the latest care plan, and any equipment owned by the participant.
Step 4. Align the start date. Most handovers happen on the first of a month. Confirm there are no gaps in personal care or clinical visits during the changeover.
Step 5. Archive everything. Keep both the old and new care plan, the final statement and any handover communication. If something breaks down later you have the trail.
